Aim

The goal is to improve stroke rehabilitation at home for older adults. The focus is on enhancing self-management, an essential component of holistic home-based rehabilitation. EQL addresses a critical and urgent issue: the unmet needs of individuals rehabilitated from a stroke in a home setting.

Description

 The project focuses on integrated health services, focusing on processes that link traditional stroke treatment and rehabilitation in the hospital with rehabilitation and care in the home environment and the community. The role of social networks, the physical environment, and collective efficacy enhancing self-management and improving rehabilitation in this vulnerable population is an understudied and critical area of research. The project aims to develop an intervention that connects social support networks and the home environment and supports self-management in older people with stroke.
